Αξιολόγηση υποθρεψίας παιδιών με συγγενείς καρδιοπάθειες- περιεγχειρητικές διατροφικές πρακτικές

Οι ασθενείς με συγγενείς καρδιοπάθειες πριν το διορθωτικό χειρουργείο, κυρίως βρέφη και παιδιά, έχουν αυξημένο κίνδυνο να εμφανίσουν ελλιπή ανάπτυξη- υποθρεψία. Η υποθρεψία εμφανίζεται λόγω αυξημένων αναγκών σε ενέργεια και θρεπτικά συστατικά, μειωμένης διατροφικής πρόσληψης ή συνδυασμού των δύο παραπάνω. Η υποθρεψία και ο υποσιτισμός περί το χειρουργείο επιδιόρθωσης της συγγενούς καρδιοπάθειας σχετίζονται με δυσμενείς επιπτώσεις όπως αργή επούλωση τραυμάτων, μειωμένη αντίσταση στις λοιμώξεις, παρατεταμένο χρόνο μηχανικού αερισμού και νοσηλείας, αυξημένη θνητότητα. Μετά το χειρουργείο επιδιόρθωσης η διατροφική κατάσταση του ασθενή βελτιώνεται παράλληλα με τη γενικότερη υγεία και λειτουργικότητά του, και γίνεται ανάκτηση ανάπτυξης (catch up growth). Όσο καλύτερη είναι η διατροφική κατάσταση του ασθενή πριν το διορθωτικό χειρουργείο τόσο αυξάνεται η πιθανότητα για καλή έκβαση της επέμβασης και ταχύτερη αποκατάσταση. Σκοπός της εργασίας είναι να μελετηθούν οι παράγοντες που συμβάλλουν στην εμφάνιση υποθρεψίας των ασθενών με συγγενείς καρδιοπάθειες πριν το χειρουργείο, να εξεταστεί η σημασία- αναγκαιότητα παροχής κατάλληλης διατροφικής φροντίδας περιεγχειρητικά, να αναδειχθούν περιεγχειρητικές διατροφικές πρακτικές ώστε να υποστηρίζεται κατά το βέλτιστο βαθμό η πορεία υγείας του ασθενή και η αξιολόγηση διατροφικής κατάστασης παιδιών με συγγενείς καρδιοπάθειες πριν το διορθωτικό χειρουργείο 0-14 ετών ελληνικού πληθυσμού. Με βάση τα πρότυπα αναφοράς που αναπτύχθηκαν από τον Παγκόσμιο Οργανισμό Υγείας, αξιολογήθηκε το ύψος προς ηλικία για άτομα 0-14 ετών, το βάρος προς ηλικία για άτομα 0-10 ετών και ο Δείκτης Μάζας Σώματος για άτομα 10-14 ετών από δείγμα μεγέθους 35 ατόμων 0-14 ετών. Από την αξιολόγηση φάνηκε πως τα άτομα με συγγενείς καρδιοπάθειες 0-14 ετών πριν το διορθωτικό χειρουργείο δεν διατρέχουν σημαντικό κίνδυνο υποθρεψίας, οι περισσότερες σωματομετρικές παράμετροι ήταν εντός φυσιολογικών ορίων. Χρειάζεται περεταίρω έρευνα όπως μελέτη δερματικής πτυχής τρικεφάλου και μυική περίμετρος μέσου βραχίονα σε μεγαλύτερο δείγμα ώστε να διεξαχθούν πιο έγκυρα και αντιπροσωπευτικά αποτελέσματα.
Before the corrective surgery patients with Congenital Heart Diseases, mainly infants and children, are at high risk for malnutrition. Malnutrition appears due to increased energy and nutrient needs, reduced dietary intake or a combination both of the above. Perioperative malnutrition is related to morbidities such as poor wound healing, reduced infection resistance, prolonged mechanical ventilation period and hospitalization, mortality. After the corrective surgery, patients’ nutritional status improves, as their general health and total fractionality do so, and they catch up growth. The better the patients’ nutritional status is before the corrective surgery, the more likely it is for them to recover sooner and have better health overall. The thesis aims to study the factors associated with malnutrition for patients with congenital heart disease, to focus on the necessity of the perioperative nutritional support, to highlight perioperative nutritional practices and directions in order to optimally support the patients health and to assess the nutritional status of 0-14 years old patients before the corrective surgery, in Greek population. Based on World Health Organization children growth charts, height to age ratio for patients 0-14 years old, weight to age ratio for patients 0-10 years old, and Body Mass Index to age ratio for patients 10-14 years old were examined, from 35 patient sample 0-14 years old. The assessment showed that congenital heart disease patients 0-14 years old before the corrective surgery are not at high risk for malnutrition, most of their anthropometric parameters are within normal limits. Furter research is needed in order to get more accurate results. For instance, tricuspid skinfold and mid arm muscle circumference could be assessed from larger sample.
